Custom Contact Forms Pro

Custom Contact Forms Pro extends the WordPress form builder for bookings, orders, registrations, donations, waivers and agreements. Payment, pricing, conditional logic and record functions connect visitor answers to a calculated total, a signed submission or a receipt. Payment forms work without an e-commerce stack.

Build forms with drag-and-drop fields and live previews. Edit field labels, set widths, pick an included theme or add custom CSS. Place a saved form with a Gutenberg block, shortcode, widget or PHP function.

When visitor choices change the total

Set a fixed price, offer options or let visitors choose an amount. Pricing fields can include quantities and images, with enforced minimums. Conditional logic can display or conceal products and priced fields based on answers; percentage or fixed-amount coupons and live totals reflect those selections.

Stripe payments stay in the form and support cards, Apple Pay, Google Pay and Link. A live payment button can display the amount due, and totals are verified on the server. PayPal payments also happen in the form with server-verified totals. The extension adds no transaction fees beyond Stripe or PayPal's standard processing fees.

Signed submissions and payment records

The form builder includes a signature field visitors can complete with a mouse, finger or stylus. The signature is saved as an picture with the submission and included in notification emails. Pro can include signatures and consent records in PDF receipts. A consent or terms field can show scrollable terms, record versioned agreements and permanently snapshot the agreement.

PDF receipts can be attached to notification emails for paid orders. Administrators can also download individual submissions as PDFs with one click.

Multi-step forms and follow-up

Break a form into steps with page breaks, progress indicators and per-step validation. Conditional logic can show or hide fields based on answers, including fields that affect pricing. Survey and star-rating fields support Likert grids and quick ratings.

Address autocomplete lets visitors select an address as they type and fills the street, city, state and ZIP fields. It uses your own Google API key. SMS confirmations can reach customers when they book or pay through your own Twilio account. Additional email marketing integrations include MailerLite, Brevo, Kit, ActiveCampaign and MailPoet, alongside the builder's Mailchimp connection.

Thirteen ready-made templates cover donations, orders, registrations, waivers, agreements and other uses. Start with a template or a blank form, arrange fields and configure the features you need. Save the form, then place it with a Gutenberg block, shortcode, widget or PHP function.

The free builder's AI form-building option uses the provider already connected under Settings > Connectors. It requires WordPress 7.0 or later and stays hidden on earlier versions.
