Survey Maker Pro

Survey Maker Pro extends Survey Maker by AYS for creating surveys, questionnaires, and forms in WordPress. The builder supports unlimited surveys, questions, and sections. Arrange questions across pages by dragging them into place, customize the styling and settings, then publish with a shortcode or the Survey Maker block. Popup surveys provide another way to present a form.

Question formats and response paths

Pro question types include Date, Time, Date and Time, Star Rating, Net Promoter Score (NPS), Matrix, Ranking, Linear Scale, Slider, File Upload, Hidden, HTML, Phone, and List. Name and Email fields are available too.

• Logic Jump chooses the next question according to a respondent’s answer.
• Conditional Results can set result pages, email recipients, or redirects using AND/OR conditions. Conditional Mailing lets you configure mailing behavior around survey conditions.
• Calculations can use answers in formulas to display scores or estimated values. URL parameters provide another control for survey interactions.
• Conversational surveys use a chat-style format. Pro also lists multilingual forms, text to speech, and ChatGPT integration.

Integrations and results

Configure email notifications for respondents and administrators, with customizable content and response data. Administrators can also create survey summary emails sent after predefined intervals. Listed integrations include Mailchimp, Slack, Zapier, Google Sheets, WooCommerce, Stripe, and PayPal.

Review submissions, statistics, and charts in the WordPress dashboard. Pro supports exporting results and submissions in formats such as CSV and PDF for use outside the website.

Participation controls

Pro includes survey scheduling, attempt limits, user roles and permissions, password protection, and user history. The base extension also supports required questions, redirects after submission, and submission restrictions by IP address, user role, or submission count.

Create and publish

1. Add questions and sections in the survey builder.
2. Customize the survey settings and design.
3. Configure question types, logic, notifications, or integrations as needed.
4. Publish the survey on a post or page with its shortcode or the Survey Maker block.

The shortcode format is [ays_survey id='your survey id']. Popup surveys can be triggered on page load, on click, or on exit intent; options include showing one after a response and using full-screen mode. After publishing, review submissions and charts in the dashboard, then export results when needed.
