SureForms Business extends the SureForms WordPress form builder with workflows for account access, feedback, assessments, calculations, and guided submissions. Build forms in the WordPress block editor, starting with an AI-generated draft, a template, or a blank form. Describe what you need in plain language and AI creates a structure with suggested fields and labels. You can edit the result as WordPress blocks, and AI form generation does not require an external API key.

Arrange, duplicate, and style fields in Gutenberg, then publish a form with a block or shortcode. The Business edition includes advanced form types and tools for more involved submission flows.

Choose a form workflow

• Account forms: Create registration, login, lost-password, and reset-password forms.
• Surveys and polls: Collect feedback, including Net Promoter Score responses, and review reports.
• Quizzes: Build scored quizzes, graded assessments, and personality tests with result output.
• Calculators: Create quote and pricing forms, BMI calculators, cost estimates, and custom formulas with real-time values.
• Multi-step forms: Divide longer submissions into stages with progress indicators.
• Conversational forms: Present one question at a time in a guided flow.

Conditional logic can show or conceal fields and control notifications or confirmation messages according to a user’s responses.

Add fields and submission controls

Business adds fields such as date and time pickers, file uploads, signatures, ratings, range sliders, repeaters, hidden fields, custom HTML, and password fields. Other tools include PDF generation, save and resume, partial entries, form restrictions, dynamic options, and custom post creation from submissions.

PayPal is available for payment forms, donations, memberships, bookings, and registration payments. SureForms also works with Stripe payment forms. Entries are managed in the WordPress dashboard, where you can review, search, and export submissions. Customize email recipients, reply-to addresses, subjects, message content, and included form data. After submission, show a confirmation message or redirect the visitor to another page.

Create and publish

Open the form builder in the WordPress block editor, then select AI generation, a template, or a blank form. Set up the fields and workflow, configure notifications and payment settings where needed, and publish with a block or shortcode.
